Jury find actions of Prison and Community Psychiatric Team inadequate in Luke Johnson's death

Victoria Butler-Cole, instructed by Liberty, represented the family of Luke Johnson, a vulnerable man with mental health problems who died as a result of taking heroin a week after being released from prison at an inquest into his death on 6-8 June 2016.

The jury returned a narrative conclusion and found that the actions of the prison and community psychiatric teams were inadequate, and that the lack of communication between the agencies involved in Luke's care contributed to his death.
